Student Driver Waiver

Driving on campus is a privilege for students. For students to drive and park on campus they must follow these steps.

Complete the online form including submitting a picture of your license and insurance.
Print and complete the waiver and liability form and turn into the office or email it to lifeao@lifechurchflorida.com

Once these two steps are submitted students will receive a driving decal from the office. The security officer will ensure all student drivers follow the student driver rules mentioned in the form.
